In order to be a coach, participatein sports program and get paid, every coach must fill out three forms, work verification
form, an official roster, and a time sheet.
The work verification and coach registration is no longer done in the leagueminder. Please download a copy from
here, fill it out on the PC and make sure the Area manager gets it before due date for each season.
No hand written or partially filled work verification forms will be accepted. No faxes will be accepted.
Forms must be handed in or received via mail run by the deadline.
